MISSION STATEMENT: The Parkland Community Living and Supports Society exists to improve the quality of life of the disabled through individual choice, dignity and rights.



  1. PURPOSE OF POSITION:
    Under the general direction of the Community Bridging Facilitator, the Community Bridger: facilitates and supports the consumers in becoming aware of the community; facilitates increasing inclusion of consumers into the community; facilitates and supports the consumers in making informed choices; encourages and facilitates the making and maintaining of friendships within the community by the consumer; encourages and supports the consumer in developing leisure activities; contributes to and supports the implementation of annual service plans and individual training plans for each consumer; completes various logs, expense claims, time sheets, and reports.
  2. TYPICAL DUTIES AND RESPONSIBILITIES:
    Under the general direction of the Community Bridging Facilitator, the incumbent in this position will be required to:
    1. Facilitate and support the consumers in becoming aware of the community by:
      • providing the consumer with information regarding the roles, rights, expectations and benefits of being a part of the local community
      • arranging and supporting the consumer in opportunities to become familiar with and involved in the community
      • encouraging and supporting the consumer in exploring the community
      • providing the consumer with a broad spectrum of community experiences including the various cultures, amenities, facilities, etc.
    2. Facilitate increasing inclusion of consumers into the community by:
      • working with the consumer to identify the breadth of choices that are available for inclusion into the community
      • assisting the consumer in identifying his/her the needs, desires, and goals
      • identifying the approaches, resources, opportunities etc. that are available in meeting these needs, desires, etc.
      • explaining and assisting the consumer in meeting community expectations
      • introducing and supporting the consumers in meeting and relating appropriately to the providers of activities services, etc.
      • assisting and supporting the consumers in the recognition, development and practice of appropriate social skills and behaviours
      • ensuring that the community is aware and open to the consumer�s inclusion
      • determining with the consumer, the level of satisfaction and/or inclusion the consumer experienced
    3. Facilitate and support the consumers in making informed choices by:
      • helping the consumer identify areas where decisions and choices can or must be made
      • encouraging and assisting the consumer in gathering information
      • instructing and assisting the consumer in the development of options and alternatives
      • helping the consumer evaluate the alternative choices and options
      • assisting the consumer in evaluating the results and modifying future decisions
    4. Encourage and facilitate the making and maintaining of friendships within the community by the consumer by:
      • providing opportunities for the consumer to meet appropriate individuals
      • assisting the consumer in recognizing the variety of relationships that are available
      • encouraging the consumer to build relationships based on the consumer�s strengths, interests, qualities, and values
      • identifying, with the consumer�s input, those relationships that are essential, those that are freedom of choice and those that are mutually beneficial
      • assisting the consumer in expanding his/her circle of friends and acquaintances
      • assisting the consumer in dealing with relationships that might have a sexual content
    5. Encourage and support the consumer in developing leisure activities by:
      • exposing the consumer to various options
      • securing the necessary approvals from family, guardian, supervisor etc.
      • encouraging and assisting the consumer in pursuit of his/her choice of activities
      • assessing the consumer�s satisfaction with the activity
      • making suggestions and working with the consumer to modify, as required, the consumer�s participation in chosen activity
    6. Contribute to and support the implementation of annual service plans and individual training plans for each consumer by:
      • participating in the development of these plans
      • identifying opportunities for attaining and/or contributing to the attainment of the goals
      • measuring progress, and reporting as required after each activity and in regular monthly and quarterly assessments
      • suggesting methods and approaches that may be more effective in implementing the plans and meeting the objectives
      • supporting other staff in their endeavours to make the consumer successful
      • maintaining open communication with the Coordinator, Facilitator and guardians
    7. Complete various logs, expense claims, time sheets, and reports by:
      • keeping notes, logs, and other records of activities
      • obtaining receipts and other required documentation to support claims, etc.
      • submitting required documents, reports, logs, etc. in a complete, accurate, and timely manner
      • attending monthly staffing meetings
  3. JOB SPECIFICATIONS:
    To be effective in the performance of the position described above, an individual would require the following:

    KNOWLEDGE:
    • Demonstrated knowledge and understanding of the values of citizenship, choice, support, and equality of opportunity, as defined and adhered to by the Parkland Community Living and Support Society
    • Detailed knowledge of the material in the Universal Precautions Handout
    • Working knowledge of First Aid and CPR
    • Demonstrated knowledge of medication administration
    • Working knowledge of behaviour modification as practised by Parkland C.L.A.S.S.
    • Working knowledge of crisis prevention and intervention
    • General knowledge of the community amenities and social activities
    SKILLS:
    • Demonstrated ability to work with consumers of Parkland C.L.A.S.S.
    • Good verbal and written communication skills
    • Demonstrated financial and record keeping skills
    • High degree of skill in interpersonal relationships
    • Skill in defusing tense and sometimes dangerous situations
    • Skill in providing a climate where consumers will be motivated to take appropriate action
    • Patience and skill in implementing training programs
    • Patience and skill in implementing training programs, chairing meetings, etc
    EDUCATION:
    To adequately perform the duties of this position the incumbent will have completed Grade 12 and have one additional year of course work that has some relevance to the goals and objectives of Parkland C.L.A.S.S.

    EXPERIENCE:
    The skills and knowledge listed above will likely have been gained through a minimum of one (1) year of experience in a related field of endeavour.

    Note: Some equivalency of experience and/or education may be accepted.

    SPECIAL REQUIREMENTS:
    • Excellent physical condition as confirmed by a medical examination
    • Clean criminal record or one that is acceptable to the Executive Director
    • Valid Class 5 Drivers Licence
    • Valid Class 4 Drivers License may be required
    • Valid First Aid Certificate
    • Valid CPR Certificate
    • Valid PART Certificate
    • CWIS (may be required)
    • Additional training may be required
